Home - Connect to Support

Residential Services

We have six services across the county – five for people primarily with learning disabilities and one for people with dementia. The homes promote people’s independence, choice and inclusion and provide support 24 hours a day. This may include support with:

  • personal care
  • mobility, fitness and well-being
  • managing health issues, including medication and attending appointments
  • communication and engagement to ensure people are included in all decisions about their care and support
  • managing money
  • going on holiday and getting out and about.

Spotlight: Tozer House film premiere!

People who live at Tozer House got together with family and friends and celebrated the showing of a film that was made about, and with them. The film which explored their experience of living in a residential home, was shown at the New Park Cinema, Chichester. It featured interviews with people living in Tozer House and members of staff and provided a great occasion for families and friends to learn about the day-to-day activites of their loved ones.

Shared Lives

Our Shared Lives Scheme has an overall CQC rating of 'Good' and an 'outstanding' rating for 'caring'. The scheme consists of over 100 households where approved, trained and highly-skilled Shared Lives carers open up their family homes to provide long or short-term care and support to people with a range of different needs, including learning or physical disabilities, mental-health needs or dementia, and vulnerable older people.  The scheme can be offered to any adult in West Sussex with assessed care and support needs and in some cases, to young people from the age of 16 years.

Shared Lives Plus Awards 2022: Human rights award for free expression

We are delighted that two people from our Shared Lives scheme received the human rights award for free expression. This is awarded to a person supported by a Shared Lives carer for speaking their mind, growing in confidence and living the life they choose.
