Proactive Care+ is a service provided jointly by your GP surgery and local community health and social care providers. This multi-disciplinary team will work in a coordinated way, alongside you and your GP, to ensure that you stay as healthy as possible and live independently at home for as long as you are able.

We know that when you are unwell or need support, you usually want to be cared for where you live, surrounded by the things and people familiar to you. Your local health and social care services also recognise this and will work together to try to help make this happen.
We do this by, amongst other things, working with you to identify your needs, minimising your risk of falling, ensuring you are taking the correct medication and/or making sure you have the right equipment to maintain your health and wellbeing.
We will also try to ensure that if you do become unwell, all of the right people can be quickly brought together to provide the right care, treatment and advice. Where you and the team feel it is appropriate, we would also like to work with you to develop a plan for what you and the team feel should happen if you become more unwell, this will be called your Anticipatory Care Plan.
