Support planning is completed in partnership with the young person to work out what support will meet their needs and the outcomes they wish to achieve.
Support planning may involve linking the young person to community resources that have not yet been considered, through professional support or provision of either residential or non-residential care.
Before the council agrees to fund any service, it will consider all available options to meet need. This is done by an internal funding panel.
This follows the 'community-led support' approach which involves working with people to identify their strengths, capabilities and existing support networks to help to prevent or delay the development of their social care needs.
Although we work with the young person and their family, we cannot always provide the service that they request and will always explore the alternatives available with them.
