- What does the Lifelong Services Transition Team do?
There are two Transition Teams in Lifelong Services and they have a specialist role in supporting young people.
1. Lifelong Services Transition Northern and Southern – This team works with eligible young people who have a learning disability and who live in the northern and southern parts of West Sussex.
2. Lifelong Services Transition Western – This team works with eligible young people who have a learning disability and who live in the western part of West Sussex. They also support eligible young people across West Sussex who do not have a learning disability. These people typically have autism or physical disabilities.
Each team has a team manager and a senior social worker practitioner and consists of social workers, care managers, and assistant care managers.
The teams will assess the eligible needs of the young person, alongside their family and carers, and plan how they can be supported to live the life they want to lead.
This may include providing specialist advice and guidance, signposting, identifying suitable community-led support, support planning and then reviewing the package of support.
We work alongside health and social care professionals, Children's Services, education, providers and commissioners to achieve person-centred outcomes.
