Guide to Lifelong Services – Transition Team

This section will explain what happens when you move on from having support from Children's Services to Adults' Services support. At the county council, we call this process 'transitioning' and the team that will be supporting you is called the Lifelong Services Transitions Team.

There is a lot to consider about transition, so we recommend that young people are supported to prepare for adulthood as early as possible.
