What is the Discharge to Assess Reablement Service?

The Discharge to Assess Reablement Service is designed to support you to leave hospital and continue your recovery and reablement in a setting in the community, with 24 hour support available.

Reablement is the name we give to short-term support designed to help you keep or re-learn skills following a period of ill health, on discharge from hospital, or if you are having difficulties caring for yourself. It can help to improve your confidence and ability to manage everyday tasks independently and safely.

Staff working in the service will talk to you about your social care needs and your physical abilities – this is an assessment of your needs. You will then agree what goals you want to achieve with the staff who will be working with you. These goals will focus on being able to carry out daily living tasks as independently as possible; for example, getting dressed, being able to prepare meals and getting around your home. An individual plan will be agreed and the goals will be specifically tailored to your needs.

The team will help you to be as independent as possible and aims to support you to return home. The service is provided in a care home setting where there are social care workers, occupational therapists and trained care staff.

The Discharge to Assess Reablement Service is for a period of up to six weeks.

The service is currently available in several locations across the county.
